This is the standalone version of fairy tale. The cover is identifiable by a single statue on textured background with greenish coloring. A version with alternate jacket was also included in the band's [Limited Edition] set.
The release date is unknown. Given that a Tʌ́-rot tape from the same record label with product number U-005T was released in July 2000, this demotape was likely released before. (Note that the letters in the product numbers only indicate the band name, and not multiple types.)
